Case Management
Youth Services serves youth ages 11- to 21-years old and their families through our case management model. Our focus is on preventing and alleviating homelessness, with priority given to youth who are currently homeless, at high risk of homelessness or living in unstable or unsafe housing.
However, under certain circumstances you may also be eligible even if your housing is not imminently at risk.
What can Youth Services help me with?
First, you and a case manager decide what services you need. Then you meet on a regular basis either at your home, in one of our offices (Brattleboro or Bellows Falls) or in the community. Areas that can be addressed include:
- Employment and career development
- Budgeting skills and money management
- Housing
- Exploring and pursuing educational goals
- Life skills
- Building social supports
- Family skills
- Crisis intervention including emergency housing
- Referrals and coordination
- Substance abuse screening and referrals
Do I need to give up my therapist or other professional I’m already working with?
No, this program is designed to coordinate with the people you already work with.
Can I afford this program?
If Medicaid eligible, this program will be covered. We can help you apply for Medicaid. If you are not Medicaid eligible, please call for other options. We do not turn anyone away for inability to pay.
Can Youth Services help me get a job?
Yes, Youth Services can help you gain interview and employment skills and assist you in applying for jobs.
Can Youth Services help me apply for Medicaid and Food Stamps?
Yes, please talk to a Youth Services staff person.
